After selecting ROM Manager it just rebooted. Nothing was installed.
After it rebooted I looked in the /cache folder and the file was no longer there?

then u must have not been on a completely stock unmodified 181 system.

If I un-installed it does remove the recovery?

I had safe strap installed but un-installed it.

I would recommend using RSD or a utility (1.6) to get yourself back to stock .181, root with motofail and then try again. it will work but the system has to be completely unmodified. and you need root for ROM manager to side load the OTA

Yeah, but I only have a Linux machine so I can't install the RSD or utility 1.6
there is a Linux version of the utility, u might have to OTA to 181, but you gotta get to completely stock before the OTA will take.... or you will get the same result as before...

For some reason I don't appear to have a /cache folder in my main directory, any idea what I could be doing wrong? Here's what I'm seeing when I open it, show hidden files and folders is enabled.
your not in the root of the phone, that is your sdcard.

Alright got it, only problem I'm having now is when I open with ES File Explorer and click open with ROM Manager, ROM manager asks me to boot into recovery then just restarts the phone. It's like it's only looking for Clockwork Mod or something.

I have 4G off, brightness set to extremely dim and yet I dont seem to last above 40% as long as I use to in GB

The only mod I made was changing 1 setting in Radiocomn to enable GSM. I have never even rooted.
I have a ton of apps so do not want to factory reset... am I "stock enough" to side load ICS?
Also, I am hoping the ICS side load will clear any changes I made to the radio to get GSM working, as the ICS options for GSM seem better than what I currently have.

YOU MUST BE ON A STOCK .181 SYSTEM WITH ROOT.
USE UTILITY 1.6 (option 1) -
YOU MUST DOWNLOAD AND INSTALL FROM GOOGLE PLAY:ROM MANAGER FREE
ES FILE EXPLORER
OTA ROOTKEEPER
Once you have these files on your Stock RAZR running android 2.3.6 (6.12.181), rooted (motofail). ..Now once you have completed the above steps, it is time to place the 211 zip into your phones /cache directory which is in the root of the phone.
- open rom manager free, hit OK, now press the home button.
- open es file explorer, press YES, press menu, open settings, scroll down to Path settings and under Root settings: Enable
- Up to Root (path settings)
- Root Explorer (root settings)
- Mount File System (root settings)
- open ota rootkeeper, allow SU, and Protect SU
once this file has been moved into your /cache directory, and you have checked the MD5 to ensure a non-corrupt download, you may proceed!
- (MD5: 79A1EE4A992EACD1D08F98DEE5DEEBF7)
Quick check list:::::
1) Blur_Version.6.12.181.XT912.Verizon.en.US is your phones /cache directory with a matching MD5!!
2) Opened OTA rootkeeper and protected SU
**INSTALL TIME*
open ES FILE EXPLORER, locate your /cache directory and open it, then select the Blur_Version.6.12.181.XT912.Verizon.en.US.zip (now choose Open With ROM MANAGER)
phone will restart, and begin installation of the 211 OTA.
phone restarts, boot animation begins, Profit success!! Celebrate!!!
**** once your phone boots up, open app drawer; locate and open OTA Rootkeeper, " restore root "
Giving this a shot now . Keep your fingers crossed for me
