So I have been having this issue for a while, either I have 4g LTE but I can't make or receive calls, OR I am at a 3G speeds but my phone is fully functional. I was told to mess around with phone settings by calling *#*#4636#*#* but there was no avail. The way to switch between my 3G functionality, and set it to 4g is by changing from cellular band, to US band. I have also attempted to flash the latest radios with RSD and is hasnt worked. Any advise is welcome.

So, when you dial the *#*#4636#*#* no avail ?  Do you mean it didn't take you to the programming area?

Have you tried to power off, remove SIM, power on and fully boot up, power down again, insert SIM, reboot?

Had to do this today as my HD was stuck in 3g only. After that sequence, 4g returned.

Well, you tried the simplest solution.

Have you dialed *228 at any point before encountering your issues? *228 is a big no-no with 4g SIMS, so please don't.

I would try getting a new 4g SIM from VZW, it won't cost you anything if you go thru Verizon directly.

I'm afraid it's the only help I can offer, as I don't know much about playing around with the programming menu (*#*#4636#*#*).

Where did you get this information from? I have never heard that *228 is not good on 4g SIM's....in fact Verizon generally tells you to do *228 whenever you travel and get to your destination...

With a 4g SIM, the prl is auto updating, meaning you don't need to dial *228 for a forced update.

When the Thunderbolt was released, owners reported that *228 would corrupt the SIM card. Also, in the link I posted to a different xda thread, someone there mentioned that *228 made them lose 4g service.

Some other reports:

Are you running a custom ROM, or the stock ROM?  I noticed you said that you flashed some radios, so Im assuming you are not stock? If not, I recommend RSDing back to stock and trying again. I don't think its an issue with your SIM card or you wouldn't have LTE ever. If you are on Verizon then you always use CDMA for voice exclusively. Thats why you can normally be on LTE surfing the web and talking on your phone at the same time. If, however, you didn't  have CDMA enabled, then you could get LTE data, but no voice. Alternatively ,if you were locked down to CMDA2K you could have all services, but not use voice and data at the same time.

Having said all that, only CDMA 1xRTT, EvDO, and 3G use a PRL (Preferred Roaming List). On older phones Verizon and formerly ALLTEL would recommend doing the *228 call, which does an over the air programming session to send a few things, including a new PRL. The PRL basically tells your device what towers, bands, networks it can use, etc. LTE does not use the same technology. Most of the time a device is set from the factory to ignore a PRL request update (*228 call), as the LTE network takes care of roaming preferences, etc., automatically. Now then, If you have tampered with the stock ROM, modem images, etc., there is no telling what would happen when you do a *228 call.

So, you're welcome to switch out the SIM, but I don't expect it's relevant. If LTE ever works then it should be good to go. This seems more like a mode problem with your phone software/ROM.  I recommend putting it back to 100% stock and seeing if that helps.

Heres what a PRL looks like:

I had attempted to run CM11, but upon facing some major issues, amongst them the lack of 4gLTE. I decided to RSD to stock. The issue resumed. I have attempted everything except a new SIM, so that is my next and seemingly only option.
