CM12.1 Call Waiting Icon Gone

I can not switch between calls.  I can answer calls but can not switch back and forth.  There is no icon left to switch.

Doing a bit of research on your problem I see that you have already covered most of the bases with no solutions on most of the popular forums.  You might want to try a different lolipop rom and gapps and see if it is still there. You can always put cm12 back on if it doesn't help.  There are good things being said about roms hosted on this forum if your intrested in trying out a different flavor.

I think I am going to have to try something different if I go back to 5/21 which is before the look of the icons changed the call waiting switch icon is there.  I can answer an incoming call but I dont have anyway to switch between calls.  So far my solution is to answer new call, talk quickly ask new called to hang up, and then I am switched back to original caller.
