I may need to send my phone in to warranty it due to slight physical damage to it. I am unlocked / rooted, is it possible to lock the bootloader?

No, you had to blow a part of the hardware to unlock. Now it's permanent. This was all in the op about unlocking

There have been reports both ways whether they will catch an unlocked device through the warranty service, it seems that most times they haven't from what I've seen.  As Jamm mentioned, the MotoApocalypse OP clearly states the answer to the question.  I do suggest that, if you can, restore your device to factory before sending it in.  Chances are that if it is a hardware fix,  they won't go into APFastboot if they don't have to and if you take it to factory, they shouldn't see the need.  But then again there is that chance.....
