Rooted, not unlocked. No safestrap. Tried to follow the manual process shown here
and now it says "Fastboot Reason: Flash Failure". I tried holding the power and volume buttons down to no avail. Please help! I thought this would be quick. Got to leave for work in less than two hours.
I even managed to dig a deeper hole but doing the software update on 7/10 so I cannot unlock bootloader.

Figured it out! Power and down button. Then all 3 buttons. I think I am done screwing around, while my phone still works.

I tried again with the manual method. The cmd window closed before i could get a screen capture on it. Just to summarize, the only thing I have successfully done on this device is rooted using Dan Rosenberg's method. The cmd window closed before i could get a screen capture on it. If you are using the manual method, there is no reason the cmd window should close on you. You should be opening the CMD window, change directory to the fastboot directory, type " fastboot flash logo logo.bin " and hit enter. It shouldn't close the window.

What you can do is just put the Logo.bin of the one you wanted in the folder where you run the one click and it will do the rest for you. BTW, I didn't see that one listed in the bootlogo archive.

As it turns out I s wrong again. Holding the buttons down let me go to normal power up but every time I turn the phone off and on it goes back to ap fastboot with flash failure. What do I do? I have only found solutions for pre latest update issues, not the current. I do not want to try something and make it worse.
The same thing happens to me on my xt926 running 9.16.9 rooted and unlocked. I tried to run the unlock bootloader warning...again... and got this: FAILED <data transfer failure <too many links>>
I'm not sure what I did and hope there is a fix for me...

I had the same problem when I was trying to root my phone using motochopper_1.2_HD. I shut down with power+down volume, and then booted using power+up volume. It takes a few times for it to work.
Then I went back and ran Motochopper again and it fixed the phone. (After I went to Verizon, and they didn't know what to do with it.)
